On  or about July 23, 2024, at the site: employees were exposed to an approximate 16 foot fall hazard, in that no fall protection (not tied off) was used while working on a 6:12 pitched roof removing and installing roof shingles.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.  On or about July 23, 2024, at the site: employees were exposed to an approximate 16 foot fall hazard when accessing and exiting the 6:12 pitched roof, in that, the side rails only extended approximately 4 inches past the roof landing.
