29 CFR  1910.184(f)(5)(i):Ten randomly distributed broken wires in one rope lay, or five broken wires in one strand in one rope lay.  On or about July 24, 2024, located at 42722 State Route 7, Clarington Ohio, 43915, the employer failed to ensure that the slings used with hand magnets were free from damage prior to use. One of the slings exhibited over ten broken wires in a single rope lay and was not tagged or removed from service.

29 CFR  1910.184(f)(5)(iii):Kinking, crushing, bird caging or any other damage resulting in distortion of the wire rope structure.  On or about July 24, 2024, located at 42722 State Route 7, Clarington Ohio, 43915, the employer failed to ensure that the slings used with hand magnets were free from damage prior to their use. The slings exhibited signs of fraying indicating they were damaged and not fit for safe use.

29 CFR  1910.303(b)(2):Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  On or about July 24, 2024, at the address of 42722 State Route 7, Clarington, Ohio, 43915, it was observed that the employer failed to comply with the manufacturer's instructions regarding the use of GFCI extension cords by ensuring the cord was attached directly to an outlet as indicated on the attached labels.
