29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) SECNY Federal Credit Union, 3819 Brewerton Road, North Syracuse, NY  13212, accessing the roof.  On or about 8/5/24: Workers were using an extension ladder to access the ledge before climbing over a mansard parapet.  This method was a two step process where workers were exposed to fall when climbing over or sliding down the sloped mansard surface before reaching the ladder.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) SECNY Federal Credit Union, 3819 Brewerton Road, North Syracuse, NY  13212, on the roof.  On or about 8/5/24: Workers were performing roofing work  on a low slope roof with a ground to eave height of approximately 17 feet, without fall protection of any kind.

29 CFR  1926.405(g)(1)(iii)(C):Flexible cords and cables were run through doorways, windows, or similar openings:    a) SECNY Federal Credit Union, 3819 Brewerton Road, North Syracuse, NY  13212, in supplying power to the roof work.  On or about 8/5/24: Power was supplied to the roof repair work via, an extension cord that connected to power inside the credit union, and was run in the threshold of a secured door before being run up to the roof.  The cord showed marks indicating friction from the bottom of the door along the cord surface.
