29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about July 31, 2024, at an establishment located at 3100 East Richmond Street, Shawano, Wisconsin, employees were exposed to machine hazards associated with moving parts when they engaged in troubleshooting activities on the Ultra Industries Dust Collection System with the Prater Rotary Feeder.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to troubleshoot.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.137(c)(2)(ii):  Insulating equipment shall be inspected for damage before each day's use and immediately following any incident that can reasonably be suspected of causing damage. Insulating gloves shall be given an air test, along with the inspection.                                                                                                                                                                         Note to paragraph (c)(2)(ii): ASTM F1236-96 (2012), Standard Guide for Visual Inspection of Electrical Protective Rubber Products, presents methods and techniques for the visual inspection of electrical protective equipment made of rubber. This guide also contains descriptions and photographs of irregularities that can be found in this equipment.  On August 5, 2024, at an establishment located at 3100 East Richmond Street, Shawano, Wisconsin, while performing preventative maintenance on Line 1, an air test was not conducted on the electrically rated gloves prior to being worn when testing to verify the electrical power was de-energized exposing employees to electrical shock hazards.
