The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.    a) Production area - On or about September 09, 2024, and at times prior, employees were exposed to slip hazards in that, the employer failed to ensure that floor of workroom was in a clean and dry condition when the Oil/water/chemicals were accumulated on the floor of the compound area while employees walking/working around the equipment.

29 CFR  1904.41(a)(2): Annual electronic submission of information from OSHA Form 300 Log of Work-Related Injuries and Illnesses and OSHA Form 301 Injury and Illness Incident Report by establishments with 100 or more employees in designated industries. If your establishment had 100 or more employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix B to subpart E of this part, then you must electronically submit information from OSHA Forms 300 and 301 to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.  a) Throughout the facility - On or about March 2, 2024, the employer failed to electronically submit information from their OSHA Form 300 Log of Work-Related Injuries and Illnesses and OSHA Form 301 Injury and Illness Incident Report for calendar year 2023 on or before March 2, 2024, when the establishment employed 130 employees during calendar year 2023.
