On or about August 13, 2024, at the temporary worksite located at 9047 6th St, Great Bend, KS 67530, an employee was exposed to struck-by, fall, and electrical shock hazards when operating a Genie Aerial Lift, model Z-80/60 with serial number Z8011-2924 without training. An unauthorized employee was operating the lift without adequate training before use. The employee was not informed of the location of energized overhead electrical hazards, did not have experience operating the lift, and used no body belt with a lanyard or personal fall arrest when in the lift basket. The employee contacted the overhead powerline with his hand and was electrocuted.
