29 CFR  1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):  On or about August 28, 2024, and at times before,  the employer did not require and/or provide fall protection to employees working near a hole with fall heights of approximately 10 to 18 feet. Employees were required to remove a hole cover and perform work near the open hole with no fall protection system/device.
