29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  On or about September 10, 2024, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet eight (8) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about September 10, 2024, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet above ground, were not protected from falls, thereby exposing the employees to fall hazards.  SRL Roofing, Inc. and its successors was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1502543, citation number 2, item number 1, and was affirmed as a final order on January 13, 2021, with respect to a workplace located at 3 Regent in Oak Brook, Illinois.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about September 10, 2024, at the above addressed jobsite, the portable extension ladder used to access and egress the roof did not extend at least three (3) feet above the roof's surface, thereby exposing employees to fall hazards.  SRL Roofing, Inc. and its successors was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1502543, citation number 1, item number 1, and was affirmed as a final order on January 13, 2021, with respect to a workplace located at 3 Regent in Oak Brook, Illinois.

On or about September 10, 2024, at the above addressed jobsite, employees performing residential roofing operations had been provided fall protection training; however, the employer failed to verify the training by preparing a written certification of training record.
