29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 3756 California Avenue, Pittsburgh, PA: On or about September 12, 2024, during roofing work, the employer did not ensure that the ladder used to get up/down from the roof was extended at least 3 feet above the roofline.

29 CFR 1926.20(b)(2):  A program did not exist that provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    a) Job site, 3756 California Avenue, Pittsburgh, PA: On or about September 12, 2024, during roofing work, the employer did not ensure that a system was in place to identify and correct unsafe working conditions. Deficiencies noted during the inspection included, but were not limited to, failing to uses fall protection while working at heights, working immediately next to a live service drop to the home, not using eye/face protection when operating nail guns, not extending ladders properly for gaining access to/egress from the roof and not protecting workers at ground level from having items above from dropping onto them.
