29 CFR  1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On or about September 16, 2024, the employer did not ensure that employees who are required to wear personal fall arrest systems, when operating the Crown SP 3500 Series Narrow Lift Trucks at heights of approximately 19.5 feet, were trained in the use of personal fall protection systems to include the nature and how to recognize fall hazards in the work area (per 1910.30(a)(3)(i)), the procedures to minimize fall hazards (per 1910.30(a)(3)(ii)), correct procedures for installing, inspecting, operating, maintaining fall protection systems (per 1910.30(a)(3)(iii)), and the correct use of personal fall protection systems and equipment (per 1910.30(a)(3)(iv)) when handling material approximately 19.5 feet high.

On or about September 16, 2024, the workplace hazard assessment had not been put in writing and certified with the person conducting the evaluation and the dates the hazard assessment was completed. Safety glasses, gloves, personal fall arrest system (PFAS) are required when performing different job tasks.

29 CFR 1910.141(c)(1)(i): Toilet facilities were not provided in accordance with TABLE J-1 of this Section:  On or about September 16, 2024 the employer did not supply an adequate amount of water closets for the amount of employees.  There are a total number of 34 employees at the facility with 25 males and 9 females.  There are only 2 water closets supplied and the standard requires three water closets.
