29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) On or about May 1, 2024, the employer did not ensure that employees were protected from injury in that they had employees exposed to fall hazards. Employees were conducting roofing operations on two-story residential home and were not utilizing a fall protection system. This condition exposed employees to fall hazards in excess of 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. (a) On May 1, 2024, the employer did not ensure that portable ladders used to gain access to the roof extended at least three feet above the landing surface, while engaged in roofing operations at heights greater than 10 feet. This exposed employees to fall hazards.
