29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employee(s) were exposed to mechanical rotating parts resulting in a caught in hazard. This was observed on or about October 10, 2024, at 505 South River Avenue, Exeter, Nebraska 68351. Employees were using the Ellis drill press without a guard.

29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Employee(s) were exposed to electrical shock hazards. This was observed on or about October 11, 2024, at 505 South River Avenue, Exeter, Nebraska 68351.  An electrical box, which was intended for use while permanently fixed to a wall and which had pre-punched knockouts, was attached to the end of a flexible cord and was used to power the Millermatic 355.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:                                                               Employee(s) were exposed to electrical shock hazards. This was observed on or about October 11, 2024, at 505 South River Avenue, Exeter, Nebraska 68351. A flexible cord used to power the DeWalt hand sander did not have strain relief.
