29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) Store 4187, Willow grove - On or about September 27, 2024, the employer did not report an employee in-patient hospitalization to OSHA within 24 hours.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1743293, citation number 1, item number 1, and was affirmed as a final order on August 13, 2024, with respect to a workplace located at 170 Highland Park Drive Bloomfield, CT 06002.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1658329, citation number 1, item number 1, and was affirmed as a final order on September 14, 2023, with respect to a workplace located at 60 Stockwell Drive Avon MA 02322.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1440630, citation number 2, item number 1, and was affirmed as a final order on April 13, 2020, with respect to a workplace located at 1900 W State Rd 426 Oviedo, FL 32765.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:   a) Store 4187, Willow grove - On or about October 9, 2024, an employee sustained a work-related injury on September 22, 2024 from a fall and the employee was hospitalized. The employer had not recorded the injury  on the OSHA 300 log.
