29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 14606 Green Castle St, Smithville MO -- employees engaged in roofing duties using pneumatic nail guns and other small hand tools were not wearing eye protection. This exposed the employees to getting shrapnel and/or debris in their eyes causing injury. This was documented on or about 10/17/2024.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential worksite located at 14606 Green Castle St, Smithville MO -- employees engaged in roofing duties were using an extension ladder to gain access to the roof. The ladder did not extend 3 feet above the landing surface (the roof being the landing surface). This exposed employees to fall hazards causing serious injury and/or death. This was documented on or about 10/17/2024.
