Newark, New Jersey 07112. a) Employees performing work activities at a residential construction site used a blue Werner Extension Ladder and were working approximately 12 feet above the ground. The ladders were not inspected by a competent person for visible defects before and after their usage. Violation occurred on or around November 20, 2024

Newark, New Jersey 07112. a) Employees using a Werner Extension Ladder for work activities on a residential construction project were exposed to fall hazards of approximately 12 feet while using the ladder with a damaged foot pad. Violation occurred on or around November 20, 2024

Newark, New Jersey 07112. a) The employer did not secure poles for a Pump Jack Scaffold by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. Employees used this scaffold to paint the exterior of a residential building under construction and were exposed to fall hazards of approximately 20 feet high during such operations. Violation occurred on or around October 21, 2024
