29 CFR 1926.102(a)(1):29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about October 24, 2024, employees engaged in residential framing operations located at 19260 Gold Street, Omaha, Nebraska 68130 were exposed to struck-by hazards in that the employer failed to ensure employees were wearing eye protection when necessary.  Instances include but are not limited to:    a)	An employee was observed cutting a piece of lumber with a portable saw without using eye protection.  b)	An employee was observed using a pneumatic nail gun without eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about October 24th , 2024, employees engaged in residential framing operations located at 19260 Gold Street, Omaha, Nebraska 68130 were exposed to fall hazards in that employees were observed working at heights greater than 11 feet without any form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 25th, 2024, employees engaged in residential framing operations located at 19260 Gold Street, Omaha, Nebraska 68130 were exposed to fall hazards in that employee were using an extension ladder that was positioned on the side of the residential structure for access and egress to and from the roof. The extension ladder did not extend at least 3ft above the upper landing surface.
