29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  Front of the House: An Employee was placing wall tile from a scaffold that was not fully planked. Employees were exposed to fall hazards.  The 5M Construction Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1746444, citation number 1, item number 2 and was affirmed as a final order on 8/14/2024, with respect to a workplace located at Tun Jose Leon Guerrero Road.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section.  On the Second floor: Employees used a Milwaukee grinder to cut tile without any control measures implemented from Table 1-Specified Exposure Control Methods When Working with Materials Containing Crystalline Silica. Employees were exposed to respiratory hazards.  The 5M Construction Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1153(c)(1), which was contained in OSHA inspection number 1746444, citation number 1, item number 4 and was affirmed as a final order on 8/14/2024, with respect to a workplace located at Tun Jose Leon Guerrero Road.

29 CFR  1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  Left of the building: An extension cord with a missing ground pin was used to provide power to a Hilti Hammer Drill. Employees were exposed to fire and electrical hazards.
