29 CFR 1910.67(c)(2)(v): A body belt was not worn, and lanyard was not attached to the boom or basket when working from an aerial lift:  On or about November 4, 2024, the employer did not ensure that employees at a jobsite located at 3742 Brookwall Dr Ste 10, Akron, OH 44333 were protected from falling 25 feet while working within the basket of a truck mounted articulating boom without wearing a personal fall arrest or travel restraint system.
