29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) On or about November 6, 2024, at the job site located at 127 Lookout Drive, Pewaukee WI 53072, the employer had not ensured that employees tasked with replacing sewer pipe had adequate protective system(s) in place prior to entering a 8 foot trench, exposing the employees to engulfment hazards.
