29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.) At the worksite, located at the intersection of Cobalt Court and Monterrey Avenue Cape Coral, FL 33904; On or about 11/8/24, the employer exposed employees to struck by hazards, in that, employees were not provided hard hats while conducting utility pole installation operations.

29 CFR  1926.454(a):The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards as specified in paragraphs 454(a)(1)-454(a)(5) of this section:  a.) At the worksite, located at the intersection of Cobalt Court and Monterrey Avenue Cape Coral, FL 33904; On or about 11/8/24, the employer exposed employees to struck by hazards, in that, employees were not provided with training on hazards associated with aerial lifts while conducting utility pole installation operations with a 2001 Isuzu NQR Altec utility aerial bucket lift truck.
