29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a)Top plate, first and second level: On or about November 13, 2024, the employer exposed employees to struck-by hazards, in that protective eyewear was not provided to employees while operating nail guns at 125 PSI, hand-held circular saws and reciprocating saws, while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a)Top plate: On or about November 13, 2024, and at times prior, the employer exposed employees to fall hazards of approximately 21 feet to the lower level, in that a fall protection system was not provided to employees engaged in framing activities using the top plate as a walking/working surface.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about November 13, 2024, the employer exposed employees to fall hazards, in that employees were utilizing an extension ladder to access the upper level that did not extend at least 3-feet past the landing which measured at approximately 12-feet, 6-inches to the lower level.
