29 CFR  1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches.  (a) M/V Fisher - On or about November 26, 2024,  and at times prior thereto, employees performing ship repair activities were exposed to fall hazards of approximately 62" to the steel deck below. This occurred as employees were working near flush manholes and other small openings that were not guarded to prevent falls.
