29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about November 6, 2024, the employer did not ensure that machine specific procedures for the 8120 tire buffer were utilized prior to service and maintenance on the equipment taking place. An employee was performing a rasp blade change without utilizing the procedures to isolate the hazardous energy sources present on the equipment (mechanical, electrical, pneumatic).

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about November 6, 2024, the employer did not ensure that a periodic inspection of the energy control procedure was performed at least annually. Employees within the facility are tasked with the service, maintenance, and operation of equipment within the facility including but not limited to tire buffers, media blasters, and retreading equipment. Hazardous energy sources include but are not limited to mechanical, electrical, pneumatic, and thermal.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 6, 2024, the employer did not ensure that effective machine guarding was in place on the 8450 automatic tire buffer. At the time of inspection safety mats which were in place were not functioning to protect employees from the moving machine components of the equipment.
