Instance B  On or about November 11, 2024 the employer did not ensure that the point of operation created at the buffing rasp blades was adequately guarded. Employees blowing down the Matteuzzi 98 E Buffing Machine (model: RAS 98E Serial No. 909) were exposed to laceration and amputation hazards in that the pneumatic arm guard failed in the dropped position, exposing the rotating buffer rasp blades.
