29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  Wonderland Tire Company located at 975 Navajo Drive in Bluffton, Ohio;  On or about November 8, 2024 the employer did not ensure periodic inspections of energy control procedures were performed to ensure that lockout/tagout procedures and requirement of 29 CFR 1910.147 were being performed. Employees were exposed to injuries such as but not limited to, lacerations and amputations in that the equipment was not locked out prior to changing out the bladed rasp wheel on the Matteuzzi 98 E Buffing Machine.

29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  Wonderland Tire Company located at 975 Navajo Drive in Bluffton, Ohio;  On or about November 8, 2024 the employer did not ensure that employees who set up and maintain the Matteuzzi 98 E Buffing Machine were trained in the skills required for the safe application, usage and removal of energy controls. Employees were exposed to injuries such as but not limited to, lacerations and amputations in that the equipment was not locked out prior to changing out the bladed rasp wheel on the buffing machine.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  Wonderland Tire Company located at 975 Navajo Drive in Bluffton, Ohio;  On or about November 8, 2024 employees were exposed to injuries such as but not limited to, lacerations and amputations in that the equipment was not locked out prior to changing out the bladed rasp wheel on the Matteuzzi 98 E Buffing Machine.  The employer did not ensure that energy control application steps were implemented to control the equipment's electrical, pneumatic, and hydraulic energy sources prior to set up activity on the machine-the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished.

Operators working near and around the buffing machine were exposed to hazards associated with nip points and a rotating  wheel as the machine removes the tread from the surface of tires.

29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  Wonderland Tire Company located at 975 Navajo Drive in Bluffton, Ohio;  Instance A  On or about November 11, 2024 the employer did not ensure that the point of operation created at the buffing rasp blades was adequately guarded. Employees operating the Matteuzzi 98 E Buffing Machine (model: RAS 98E Serial No. 909) were exposed to laceration and amputation hazards in that the gaps between the guard and the blade housing were large enough to allow access to the buffing rasp blade while in the upright position.  Instance B  On or about November 11, 2024 the employer did not ensure that the point of operation created at the buffing rasp blades was adequately guarded. Employees blowing down the Matteuzzi 98 E Buffing Machine (model: RAS 98E Serial No. 909) were exposed to laceration and amputation hazards in that the pneumatic arm guard failed in the dropped position, exposing the rotating buffer rasp blades.

The descriptions in column F lacked the required information, including but not limited to, the description of the injury or illness and the object/substance that directly injured or made the employee ill.
