29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about December 6, 2024, at a jobsite located at 221 Kiddle Ln., Union Grove, WI 53182, employees were operating pneumatic nail guns installing roof shingles without using eye protection. This exposed the employees to a struck-by and eye impalement hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about December 6, 2024,  at a jobsite located at 221 Kiddle Ln. Union Grove WI, 53182, employees conducting roofing activities (paper and shingle installation) on a 6:12 pitch  new construction residential roof were not protected from falling to the lower level. This exposed employees to a fall hazard  of approximately 11.3 feet on the South and North side of the structure, and 25 feet from the gabled ridge on the West and East sides of the structure.
