29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a) On or about April 24, 2025,  at a new construction residential jobsite, located at 1221 W Kiddle Ln. Union Grove, WI 53182, the two extension ladders being used by employees to access the roof of the structure at or near the front door did not extend at least three feet above the upper point of contact with the eave nor was it secured in place. This exposed employees accessing and egressing the roof to a fall hazard of approximately  12.7 feet onto gravel and building materials.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about April 24, 2025,  at a jobsite located at 1221 W. Kiddle Ln., Union Grove, WI 53182 , employees conducting roofing activities (paper and shingle installation) on a 6:12 pitch  new construction residential roof were not protected from falling to the lower level. This exposed employees to a fall hazard  of approximately 12.7 feet onto gravel and building materials.  Alvarez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1791967, citation number 1, item number 2 and was affirmed as a final order on April 21, 2025 with respect to a workplace located at 221 Kiddle Ln. Union Grove WI, 53182.
