29 CFR 1910.23(c)(4):The employer did not ensure that ladders that are used only on stable and level surfaces unless they are secured or stabilized to prevent accidental displacement. On or about December 5, 2024, on the 3rd floor of roof an employee was in the process of climbing an extension ladder to access an exhaust fan that was not used on stable and level surface nor secured or stabilized to prevent accidental displacement.
