29 CFR 1904.40(a):The employer did not provide an authorized government representative the records within the four business hours.  a) 111 Valley Road, Hamburg, PA, on or about December 13, 2024, OSHA 300 & 300A forms were requested from the employer by an authorized government representative on December 13, 2024 and the forms were not provide to the authorized government representative by the employer until January 3, 2025.  No abatement certification or documentation required.

29 CFR 1910.95(i)(4):  The employer did not provide training in the use and care of all hearing protectors provided to employees:  a) 111 Valley Road, Hamburg, PA, machine operations, employees with an exposure above 85 dBA that have not received a baseline audiogram have not been provided training on the use and care of hearing protection, on or about January 28, 2025.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
