Douglas Street, Philadelphia, PA - On or about January 13, 2025, the employer did not ensure employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. Employees were performing roofing approximately 20 feet above ground level without the use of a fall protection system.
