The trench excavation consisted of granular cohesionless Type B soil and was approximately 9 feet deep, 250 feet long, and 5 feet wide.

Location: a) Worksite: 307 Hobart Avenue, Short Hills, NJ 07078  a) On or about 1/15/2025, employee(s) were  exposed to cave-in, crush-by, and struck-by hazards while working in a trench excavation. The trench excavation consisted of granular cohesionless Type B soil and was approximately 9 feet deep, 250 feet long, and 5 feet wide. No protection was provided to prevent materials from falling or rolling into the excavation. The excavated materials were kept within 2 feet of the edge of the excavation.

Location: a) Worksite: 307 Hobart Avenue, Short Hills, NJ 07078  a) On or about 1/15/2025, employee(s) were exposed to cave-in, crush-by, and struck-by hazards while working in a trench excavation at the worksite. There was no designated competent person at the worksite to perform inspections of the excavation, the adjacent areas, and protective systems. The trench excavation consisted of granular cohesionless Type B soil and was approximately 9 feet deep, 250 feet long, and 5 feet wide. Employee(s) did not have an adequate protective system in the trench excavation and excavated materials were kept at the excavation edge.
