29 CFR  1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a) Molding Department- On or about January 17, 2025, employees working with the air pressure machine "A" were required to make adjustments to the machine at the point of operation. The employer did not ensure that point of operation guards were designed and constructed as to prevent the operator from having any part of their body in the danger zone, when the guard was broken, and falling off.   b) Molding Department- On or about January 17, 2025, employees working with the air pressure machine "A47" were required to make adjustments to the machine at the point of operation. The employer did not ensure that point of operation guards were designed and constructed as to prevent the operator from having any part of their body in the danger zone, when the guard was broken, missing 2 screws.  c) In the Molding Department -On or about January 17, 2025, the employer did not ensure that all guards on machines, such as air press machine "A38", were completely affixed and/or secured."

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  a) In the Molding Department- On or about January 17, 2025, the employer did not ensure that all guards on machines such as air press machine "A38", were affixed and/or secured in a manner that would prevent employees from having any part of their body in the machine moving parts or danger zone during the operating cycle. The guard on the air press machine was not secured or interlocked to prevent employees from accessing the area during normal production.  Grier Abrasive Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(2), which was contained in OSHA inspection  number 1742484, citation number 1, item number 1 and was affirmed as a final order on June 6, 2024, with respect to the workplace located at 123 Taft Drive, South Holland, IL 60473.
