29 CFR 1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces:  (a) Building 1; On or about, January 30, 2025, and dates prior, a section of the bottom stair tread measuring 6-inch deep by 52.5 inch wide was missing and/or not installed, so safe access to and egress from the upper platform on a custom-made metal stairway with integrated platform was not available for the employee who used it to perform elevated work activities at a Lagun (FTV-4) vertical mill.

29 CFR 1910.25(b)(3):  The employer did not ensure that stairs have uniform riser heights and tread depths between landings:  (a) Building 1; On or about, January 30, 2025, and dates prior, riser heights and stair tread depths were not uniform on a custom-made metal stairway with integrated platform, that was used to perform elevated work activities at a Lagun (FTV-4) vertical mill.  The riser height varied from 12.5 inches, to 16.75 inches, to 12.75 inches and the stair tread depth varied from 16.5 inches to 12.25 inches.

29 CFR 1910.25(b)(4): The employer did not ensure that stairway landings and platforms are at least the width of the stair and at least 30 inches (76 cm) in depth, as measured in the direction of travel:  (a) Building 1; On or about, January 30, 2025, and dates prior, the platform integrated into the top of  a custom-made metal stairway was only 24 inches deep while an employee stood on it to perform elevated work activities of drilling and countersinking holes at a Lagun (FTV-4) vertical mill.

29 CFR 1910.25(c)(2): The employer did not ensure that standard stairs have a maximum riser height of 9.5 inches (24 cm):   (a) Building 1; On or about, January 30, 2025, and dates prior, a custom-made metal stairway with integrated platform, that was used to perform elevated work activities at a Lagun (FTV-4) vertical mill, had risers that were 12.5 inches, 16.75 inches, and 12.75 inches high.
