29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 13, 2025, on the west side of an excavation, a ladder was not extended 3 feet above the landing surface.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about February 13, 2025, on the west side of an excavation, employees were not protected by adequate benching while working inside the excavation.   QRO Mex was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1746948, citation number 1, item number 5 and was affirmed as a final order on (11/07/2024), with respect to a workplace located at 3440 S. Kirkner Rd. San Antonio, TX 78263.
