This condition exposed employees to crush-by hazards associated with unsafe forklift operation. (b)Forest Products Distributors, Inc. at 5400 Old Folsom Road, Rapid City, SD 57703: On or about and at times prior to February 27, 2025, the employer did not ensure that a Manitou LPG forklift, located in the Maintenance Bay building was marked to identify load and lifting capabilities. This condition exposed employees to crush-by hazards associated with unsafe forklift operation. (c)Forest Products Distributors, Inc. at 5400 Old Folsom Road, Rapid City, SD 57703: On or about and at times prior to February 27, 2025, the employer did not ensure that a Mitsubishi LPG forklift, located in the "Stacker Room" was marked to identify load and lifting capabilities.
