29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  (a) In the warehouse, located at 14651 Jetport Loop, Fort Myers, FL 33913, on or about 3/11/2025, the employer exposed employees to struck by hazards, in that the employer's training program for forklift operators did not include formal instruction in the operation of the Unicarrier MCP1F2A20LV Serial # CP1F29W26552 and Toyota 7FGCSU20 Serial # 70379 forklifts in use at the site.
