29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  The employer does not protect each employee from fall hazards while performing roofing activities on residential homes and or structures with unprotected sides and edges at height of approximately 22 feet above the ground by the use of guard rail systems, safety net systems, and or personal fall arrest systems. This most recently occurred on or about March 11, 2025, during a roof replacement on a job site located at 3706 Rocky River Dr., Cleveland, OH . Employees are exposed to the hazard of falling approximately 22 feet to the ground while working on a roof of a single-family home installing roofing material without the use of fall protection and or a fall protection system.  Lovell Construction LLC was previously cited for a violation of this occupational safety and health standard (1926.501(b)(13), which was contained in OSHA inspection number 1689410, citation number 1, item number 1 and was affirmed as a final order on 09/13/2023, with respect to a workplace located at 1373 Thoreau Rd, Lakewood, OH 44107.    Lovell Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1656531, citation number 1, item number 1 and was affirmed as a final order on 04/24/2023, with respect to a workplace located at 3748 East Royalton Rd, Brecksville, OH 44141.     Lovell Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1394075, citation number 2, item number 1 and was affirmed as a final order on 06/07/2019, with respect to a workplace located at 3710 Longwood Ave, Parma, OH 44134.
