29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about June 3, 2025, employees were exposed to struck by hazards, while performing roofing work on a residential home located at 3603 Klusner Ave, Parma Ohio 44134; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material using air driven nail guns.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  The employer does not protect each employee from fall hazards while performing roofing activities on residential homes and or structures with unprotected sides and edges at height of approximately 12 feet above the ground by the use of guard rail systems, safety net systems, and or personal fall arrest systems. This most recently occurred on or about June 3, 2025, during a roof replacement on a job site located at 3603 Klusner Ave, Parma Ohio 44134. Employees are exposed to the hazard of falling approximately 12 feet to the ground while working on a roof of a single-family home installing roofing material without the use of fall protection and or a fall protection system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 3, 2025, employees were exposed to fall hazards, while utilizing portable ladders to gain access/egress to the working surface to perform roofing work on a residential home located at 3603 Klusner Ave, Parma Ohio 44134, the employer did not ensure portable ladders extended 3 feet above the upper landing surface.  Lovell Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Ladders), which was contained in OSHA inspection number 1656531, citation number 1, item number 2 and was affirmed as a final order on 04/24/2023, with respect to a workplace located at 3748 East Royalton Rd, Brecksville, OH 44141.
