29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) Worksite -18 North 5th St., Hudson, NY - Employees were repairing a slate roof of a residential mansard building from a ladder jack scaffold approximately 20-feet above the ground. The employees were accessing the scaffold by climbing over the plank to access the support ladder. Employees did not have access to the scaffold.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) Worksite -18 North 5th St., Hudson, NY - Employees were repairing a slate roof of a residential mansard building from a ladder jack scaffold approximately 20-feet above the ground. The employees were wearing harness and had lanyards connected to anchor points but were not tied off to the lanyards. Employees were exposed to fall hazards.
