29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment.  Facility at 310 Bourne Avenue (building 50) in Rumford, RI:  a.  On or about March 24, 2025, the employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment for employees that perform work tasks, including but not limited to, spray painting and fiberglass work, thereby exposing employees to flammable organic vapors which may affect the brain or nervous system and respirable dust which may cause cumulative lung damage.  Amaral Custom Fabrications Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.132(d)(1), which was contained in OSHA inspection numbers 1670157 and 1734233 respectively, citation number 2, item number 1a and was affirmed as a final order on 12/13/2023 and citation number 3 item number 1a and was affirmed as a final order on 9/17/2024, with respect to a workplace located at 310 Bourne Avenue (building 50) in Rumford, RI.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment.  Facility at 310 Bourne Avenue (building 50) in Rumford, RI:    a.
