29 CFR  1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    37 Curtis Road Glastonbury, CT. On or about April 17, 2025 a folding "Gorilla Ladder" used to access the South side of the roof was not extended to allow the side rails to extend at least three feet above the upper landing, nor were any other safety measures to assist employees on and off the roof in place.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   37 Curtis Street Glastonbury, CT. On or about April 17, 2025, employees were not wearing fall protection nor were they otherwise protected from fall hazards while working on the 5/12 pitch roof of a single-family residence.   Express Quality Roof LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1774841, citation number 1, item number 2 and was affirmed as a final order on February 18, 2025, with respect to a workplace located at 16 West Road Ellington, CT 06029.
