29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  225 Spring Street, Wethersfield, CT roofing project:  On or about October 24, 2025, employees were exposed to a fall hazard of approximately 20 feet from the eave of the roof to the ground while performing residential roofing work activities without fall protection.  Express Quality Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA Inspection Number 1818742, Citation Number 2, Item Number 1 and was affirmed as a Final Order on 06/26/2025, with respect to a residential roofing work project located on 37 Curtis Street, Glastonbury, CT.  Express Quality Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA Inspection Number 1774841, Citation Number 1, Item Number 2 and was affirmed as a Final Order on 02/18/2025, with respect to a residential roofing work project located on16 West Road, Ellington, CT.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  225 Spring Street, Wethersfield, CT roofing project:  On or about October 24, 2025, employees were using an extension ladder to access the roof and the ladders did not extend at least 3 feet past the landing exposing them to a fall hazard of approximately 20 feet.  Express Quality Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.1053(b)(1), which was contained in OSHA Inspection Number 1818742, Citation Number 1, Item Number 2 and was affirmed as a Final Order on 06/26/2025, with respect to a residential roofing project located on 37 Curtis Street, Glastonbury, CT.
