29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Location:  Facility  The employer did not establish a lockout / tagout ( LOTO ) program.  Instance A:  the employer did not develop and implement written LOTO procedures for maintenance and servicing tasks. Instance B:  employees are not locking out equipment prior to performing servicing and maintenance tasks. Instance C:  the employer did not provide LOTO training for employees performing maintenance and servicing tasks. Instance D:  the employer did not perform a periodic inspection of its LOTO program.

Instance C:   The Elu saw was inadequately guarded.
