29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 429 Ridge Road Perkasie, PA, 18944, front of residence: On or about May 19, 2025, employees were observed on a lower roof at approximately 12 feet above grade,  conducting roofing work and were not protected from falling.   b)  429 Ridge Road Perkasie, PA, 18944, front of residence: On or about May 19, 2025, employees were observed  on an upper roof approximately 20 feet above grade, conducting roofing work and were not protected from falling.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 429 Ridge Road, Perkasie, PA 18944, front of residence: On or about May 19, 2025, an extension ladder utilized by employees to access the lower roof  was not extended 3 feet (.9 m) above the upper landing surface approximately 12 feet from ground level.  The ladder was also not secured at its top to a rigid support to prevent deflection.  b) 429 Ridge Road, Perkasie, PA 18944, front of residence: On or about May 19, 2025, an extension ladder utilized by employees to access the main roof  was not extended 3 feet (.9 m) above the upper landing surface approximately 20 feet from ground level.  The ladder was also not secured at its top to a rigid support to prevent deflection.
