29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   At 4121 Fortitude Ct, Florissant, MO, 63034, on May 14, 2025, Employees were exposed to falls from ladders that did not extend at least three feet above the upper landing surface with being accessed.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  At 4121 Fortitude Ct, Florissant, MO, 63034, on May 14, 2025, employees were exposed to falls of greater than six feet while working on a residential structure due to no protection from guardrail systems, safety net system, or personal fall arrest system, no were employees provide with an alternative fall protective measure.  Juan Garcia DBA Juan Garcia Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1618190, citation number 1, item number 1 and was affirmed as a final order on October 18, 2022, with respect to a workplace located at 11990 Sheffield Drive in Florissant, Missouri.  The Juan Garcia was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1_, citation number 1, and was affirmed as a final order on July 12, 2023 with respect to a workplace located at July 12, 2023 at 2537 Netherton Drive in Saint Louis.
