29 CFR  1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work:  a) facility : Employees were using the Milwaukee bench grinder serial# W6-00 to shape metal pieces that had a missing work rest for both grinding wheels creating an opening measuring more than 1/8 inch.   Observed on 05/22/25.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) facility : Employees were using the Milwaukee  bench grinder serial# W6-00 to shape metal pieces  without a tongue guard for both grinding wheels creating an opening between the wheel and peripheral member measuring  more than 1/4 inch. Observed on 05/22/25.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR  1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:   a) facility: Employees were exposed to electrical hazards due to damaged electrical wiring on the Spartan 1W180DX punch press machine.  The  outer insulation of the power cord and the outer insulation on the foot petal cord was damaged.  On or about 05/22/25  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
