29 CFR  1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a)  Weld Shop:  On May 22, 2025 and times prior, employee operated an 8" Craftsman (Model #10.19660) 3/4 HP bench grinder with a work rest.  The work rest had an opening of approximately 1/4"between the wheel and the work rest.  (b)  Machine Shop:  On May 22, 2025 and times prior, employee operated an 8" Craftsman (Design #A561X2000) 3/4 HP pedestal abrasive wheel grinder equipped with a work rest.  The work rest had an opening of  approximately 1/2" between the wheel and the work rest.  No abatement certification or documentation required.  The Flying W Plastics Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(a)(4), which was contained in OSHA inspection number June 23, 2021, citation number 1538357, item number 2-a and was affirmed as a final order on December 1, 2021, with respect to a workplace located at 487 Vanhorn Drive, Glenville, WV  26351.

29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:  (a)  Weld Shop:  On May 22, 2025 and times prior, employee operated an 8" Craftsman 3/4 HP (Model #10.19660) bench abrasive wheel grinder equipped with a tongue guard.  The tongue guard had a maximum opening of approximately 1-5/8".     (b)  Machine Shop:  On May 22, 2025 and times prior, employee operated an 8" Craftsman (Design #A561X2000) 3/4 HP pedestal abrasive wheel grinder equipped with a tongue guard.  The tongue guard had a maximum opening of approximately 3/4".   No abatement certification or documentation required.  The Flying W Plastics Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(b)(9), which was contained in OSHA inspection number June 23, 2021, citation number 1538357, item number 2-b and was affirmed as a final order on December 1, 2021, with respect to a workplace located at 487 Vanhorn Drive, Glenville, WV  26351.

29 CFR  1910.29(f)(4):  Openings in stair rails exceeded 19 inches (48 cm) at its least dimension:  (a)  Warehouse dock:  On May 22, 2025 and times prior, an employee used the stairway to access the loading dock behind the warehouse.  The stair rail system was not equipped with a mid rail resulting in an opening of approximately 24 inches between top hand rail and the bottom hand rail.  No abatement certification or documentation required.  The Flying W Plastics Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.29(f)(4), which was contained in OSHA inspection number June 23, 2021, citation number 1538357, item number 1 and was affirmed as a final order on December 1, 2021, with respect to a workplace located at 487 Vanhorn Drive, Glenville, WV  26351.
