1926.502(h)(1)(v)  The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: 29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest, warning line system, or safety monitor.  a. Facade Roof - On or about May 23, 2025, employees were installing insulation and rubber roofing material on the facade roof without the use of any type of fall protection, employees were working on a low slope roof, exposed to a fall hazard of 12'9" to the outside to a muddy grade.  b. Facade Roof - On or about May 23, 2025, the facade roof had multiple floor openings, near the wall of the building, employees were installing insulation and rubber roofing material on the facade roof without the use of any type of fall protection, employees were working on a low slope roof, exposed to a fall hazard of greater than 12'6" to a concrete surface.
