(a) On or about June 2, 2025, the employer did not ensure that employees installing underlayment on a residential property with a 8:12 pitch were protected from falling to the lower level, exposing employees to a fall hazard of approximately 12 feet to the ground below.

(a) On or about June 2, 2025, the employer did not ensure the extension ladder used by employees to access and egress the residential roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 12 feet to the ground below.
