29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about June 17, 2025, at N58 W25914 Autumn Way, Lisbon, WI 53089, employees installing a roof on a new residential home with a 7:12 pitch were not using fall protection. This exposed the employees to a fall hazard of approximately 21 feet from the roof eve to the ground below on the right side of the home.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) On or about June 17, 2025, at N58 W25914 Autumn Way, Lisbon, WI 53089, an employee conducting roofing activities on a 7:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  (a) On or about June 17, 2025, at N58 W25914 Autumn Way, Lisbon, WI 53089,  the employer did not ensure that employees conducting roofing operations exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.
