On or about June 12, 2025, the employer did not ensure that standard stairs were used to provide access from the floor to the grading deck in the grading shed when operations necessitated regular and routine travel from the floor onto the grading deck where an employee graded lumber 3-4 times per week.

On or about June 12, 2025, the employer did not ensure that a flight of stairs with 3 treads and 4 risers used to access a lumber grading station in the trim shed was equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.
